What version of Joomla Bible Study are you using, Paul? 6.2.x and 7.0.x work very differently when it comes to images.

The thumbnail shows up in the header - you have to choose the image in the study record itself, then in the Templates tab under StudyDetails or Studies List, you choose Thumbnail as one of the items to show in the header. Then you have to choose to show the header too (but that should be "on" by default).

Let me know if this makes sense or is not what you expected!
